Ghost/Creature Backgrounds

This is the biography that we request all ghost/creature characters try to fill out. You're always free to add more detail to the forms, but this is what is more or less expected to allow other roleplayers to be able to learn about your character quickly and effectively.
Note: As ghosts and creatures is a very broad range there are some details not on the forms which you should try to add (like death date for ghosts).

The Basic Form
